The role of the Tribal Resource Nurse will assist tribe members with in-home health services and assessments and provide resources and education for the overall health and wellness of the community.

Tuesday – 8:00 a.m. to 4:30 p.m.
Wednesday – 8:00 a.m. to 4:30 p.m.
Thursday – 8:00 a.m. to 4:30 p.m.
Friday – 8:00 a.m. to 4:30 p.m.

In-home health services, assistance with in-home telehealth appointments between you and your doctor, medication management, physical assessments (mobility, skin and wound care, diabetic foot exams), nutrition, health, and wellness education, prior authorizations for insurances, referrals, and resources to the community (vision, dental, podiatry, hearing, counseling physical and occupational therapies, caregiver support services,  +etc.).
